Before giving out ANY information, or attaching your signature to anything, make sure the agents at your door, on your land, in your barn, first fill this questionaire out.  You have a RIGHT TO KNOW who these people are, who they represent and under what authority or basis in law they claim to elicit information from you, to be used against you.  Although the agent(s) appearing may be employed by your state that employment is the result of the acceptance of federal funding: they are implementing federal business plans and will be supplying any and all information gathered from you to federal agencies routed through their offices.

PUBLIC SERVANT’S QUESTIONNAIRE

Public Law 93-579 states in part:

“The purpose of this Act is to provide certain safeguards for an individual against invasion of personal privacy by requiring Federal agencies…to permit and individual to determine what records pertaining to him are collected, maintained, used, or disseminated by such agencies.”

The following questions are based upon that act and are necessary in order that this individual may make a reasonable determination concerning divulgence of information to this agency.
